DS-11 Applicants Age 16 or Older
Are you a U.S. citizen who is age 16 or older and needs to apply for a passport with a DS-11 form?
DS-11 Applicants Age 15 or Younger
Are you the parent or guardian of a U.S. citizen who is age 15 or younger and needs to apply for your child’s passport with a DS-11 form?

You must provide one photo with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants.
For qualified applicants, the Embassy can print emergency limited-validity passports on the same day of the application. Please email HelsinkiACS@state.gov and provide your full name, date of birth, most recent passport number (if available) and a description of the nature of your emergency.
